Professional Staff

When you choose Capital Region Medical Center’s home health care, you'll get a personalized approach, high-quality, cost-effective care and access to the best clinical technology available in local home health care. CRMC’s home health care services are staffed by caring and qualified healthcare professionals including:

  • Professional nurses from Capital Region Home Health who assess your condition to assure that your health care needs are met and perform the skilled procedures ordered by your physician.
  • Occupational, Physical and Speech Therapists who provide exercises to develop body strength and coordination as well as mastery of everyday skills.
  • Medical social workers who are available to assess social and emotional factors and assist with community resource planning as well as long-range planning and decision making.
  • Home Health aides who assist with personal care including bathing, hair, nail and skin care as well as dressing. Home Health aides also take and record your vital signs.

In addition, we continually evaluate our services to ensure that we are providing the best care possible. One way we assess our performance is by conducting formal satisfaction surveys with patients and referral sources.

Telehealth Services

The Telehealth monitoring system promotes independence in management of disease process(es) at home and can help avoid rehospitalization. Telehealth is a very simply system that monitors vital signs, weight, Oxygen levels, etc., on a daily basis. Results are transmitted to the Home health office and reviewed by a nurse allowing for immediate intervention, if necessary, with patients, patient's family, and physicians. many times interventions can be accomplished via the phone (such as medication changes) and thus, reduce extra physician visits and/or rehospitalization. Telehealth provides peace of mind knowing assistance and possible intervention is only a click away. There is no extra charge for this service while receiving home health services. However for a nominal fee of $7.50 per day, Telehealth services can be provided for patients who do not quality for home health and/or have been discharged from home health and would like to continue the monitoring service.

Around-the-clock Service

Our on-call professionals help with around-the-clock service. Our staff have the education, experience and expertise to meet your needs, including specialty areas such as:

  • Physical Therapy
  • Occupational Therapy
  • Speech Therapy
  • Respiratory Therapy
  • Pediatric Therapy
  • Medical Social Work
  • Nursing Care (including intravenous therapy, injections, pain control, wound care, diet & medication teaching, as well as obstetric, newborn and pediatric services)
  • Home Health Aides (help with personal care, exercises, ambulation)
  • Patient and Caregiver Education program is provided by clinicians, to help you monitor changes in the patient's status, ensuring proper and timely treatment.
  • Durable Medical Equipment:
    • Respiratory Services – CPAP, nebulizers, respiratory medications, ventilators
    • Oxygen Therapies – stationary systems and gas or liquid portable systems
    • Infusion Therapies – antibiotics, antivirals, chemotherapy, pain management, TPN and patient education
    • Enteral Therapies – equipment, supplies, patient follow-up and education
    • Home Medical Equipment – wheelchairs, hospital beds, ambulatory aids and bathroom aids

Eligibility Requirements

To be eligible for home health care, the patient must be under a physician's care and have a physician's referral for home healthcare and/or teaching.

Payment is usually covered by Medicare/Medicaid, primary health insurance company or self pay. A home health care nurse manager can quickly verify insurance coverage, handle pre-certification and process all forms and filings to ensure valuable time isn't lost when home health care is needed.
